Cat Adoption Form 🐾 Important Notice Before Filling Out the Form We know the form is long. Adoption is not a temporary experience but a responsibility for many years. These questions help us ensure the cat will go to a safe and suitable home. If filling out the form seems bothersome to you, adoption is likely not the right choice for you at this time. The cats' best interest comes first. Notes: • Adoption within or around Riyadh • The adopter will receive the cat • The adopter must fill out the form themselves • Those initially selected will be contacted via WhatsApp First: Personal Information Name: Age: Mobile Number: Are you employed? (Yes / No) The city where the cat will live: Second: Commitment and Care What will you do with the cat when you travel? From where will you provide for the cat? What is the expected average monthly expense? Will you give the cat the necessary vaccinations? If the cat is young, will you spay/neuter it at 5–6 months of age? How much do you expect the sterilization procedure to cost? Third: Your Experience with Cats How many cats do you currently have? How many cats do you prefer to raise? Have you raised cats before? Tell us about your experience and what happened to them. Have you spayed/neutered your cats before? Fourth: Home Environment Please describe your home: (Size – presence of a yard or garden – windows – number of household members and their ages – their acceptance of animals) Where will the cat live? • Inside the house as a family member • A separate room inside the house • A separate room in the yard • The house yard • Farm / Rest house How many hours a day will the cat spend without you? Do all household members agree to the adoption? Are there children? (Their number and ages) Are there smokers or hookah users in the house? Who will be the primary caregiver for the cat? Fifth: Health Care Do you or any household member suffer from allergies to cats? How did you find out? If an allergy appears later, what will you do? Which veterinary clinic do you visit or plan to visit? What diet will you adopt for the cat? Where will you place the litter box in the house? How many times a year do you think the cat needs a bath? If the cat gets a disease that requires long-term care, what will you do? Sixth: Adoption Why do you want to adopt this cat? If you are chosen for adoption, who will come to pick up the cat? What reason might make you consider giving up the cat – God forbid? How will you introduce the cat to the new home, family members, and other animals? Finally Do you agree to contact us from time to time and send photos or videos of the cat? When you are chosen for adoption, the cat will not be handed over until its place and belongings are prepared and photographed for us. Important Notice Before Submitting the Form Please answer all questions honestly and truthfully without lying or manipulating information. Allah Almighty said in the Quran: "O you who have believed, fear Allah and be with those who are true." And Prophet Muhammad (peace be upon him) said: "Adhere to truthfulness, for truthfulness leads to righteousness, and righteousness leads to Paradise. A person may speak the truth until he is recorded as truthful with Allah. Beware of lying, for lying leads to wickedness, and wickedness leads to Hellfire." Adoption is also a trust, and the Prophet (peace be upon him) said: "Each of you is a shepherd and each of you is responsible for his flock." Therefore, we kindly ask you to fill out the form with a sincere intention and true responsibility, as the cat is a soul entrusted to you.
